General Petition Routing Process

For a General Petition

• Meet with your advisor to discuss if you should file a General Petition.
• Download the General Petition form.
• Read all instructions on the form.
• Fill out, sign, and date forms. Follow instructions for additional required documentation.
• Submit completed petition packet to petition@arizona.edu.
• Petition is reviewed by the Office of the Registrar and then the General Petition Committee.

For a General Petition for Retroactive Medical Withdrawal 

• Meet with your advisor to discuss if you should file a General Petition.
• The retroactive medical withdrawal process is now available online through UAccess Student Center.
• If you have questions or need assistance with the online process, contact the Office of the Registrar at petition@arizona.edu
• Login to UAccess Student Center
  • Go to Academic Record Tab
  • Select, Academic Summary
  • Select, Term Withdrawal
  • Select the TERM
• Follow step by step instructions.
  • Upload the Medical Provider form signed by a licensed health care provider.
  • Write or upload a personal statement.
  • Submit petition for processing.
• Petition is reviewed by the Office of the Registrar and then the General Petition Committee.

For All Petitions

• The General Petition Committee may approve the request as submitted, approve it with modifications, or deny it. A modified approval may result in an outcome different from the original request. 
• The petition decisoin is emailed to the student by the Office of the Registrar.
• If the petition is approved, changes will appear on your academic record. 
• An approved retroactive withdrawal is academic in nature only and does not remove or reduce tuition and fees on your account. To receive an automatic refund, you must drop your courses by the published Refund Dates during the term. If you missed the refund deadline due to extenuating circumstances you may submit a Tuition Appeal for review.
