General Petition for Retroactive Medical Withdrawal 

Students should follow the steps below to request a retroactive withdrawal for medical reasons.

  • Complete the Medical Provider form:
    • Download the Medical Provider form.
    • Fill in their student information at the top of the form.
    • Provide the form to their healthcare provider.
    • Their healthcare provider completes their portion of the form and returns it to the student.
  • Submit a medical withdrawal request on UAccess Student Center:
    • Use the "Academic Record" drop-down menu and choose Academic Summary.
    • Select "Retroactive Medical Withdrawal" to begin the process which will require uploading the completed Medical Provider form and personal statement.  
    • Note: Students may request a full retroactive withdrawal (all classes taken in the term) or partial retroactive withdrawal (only some classes taken in the term) using this form. Students should specify what type of withdrawal (full or partial) they are requesting in their personal statement. When requesting a Retroactive Partial Withdrawal students must provide clear evidence explaining why not all classes in the term were impacted.

Note: The Medical Provider form is the only accepted medical documentation for medical withdrawals.
 


General Petition for Extenuating Circumstances - Sexual Harassment or Discrimination (Title IX)

Students can submit the Petition for Retroactive Academic Withdrawal form for sex-based harassment or discrimination by emailing a completed form to equity@arizona.edu. Students can contact the Office of Institutional Equity (OIE) at 520-621-9449.
