UA Course Fee Forms

UA Course Fee Forms are found in UAccess Student, at the following navigation: Main Menu > Curriculum Management > UA Curriculum Management > UA Course Fee Management.

Only users with the Course Fee Request Initiator role in UAccess will be able to submit requests, for training and access follow the Initiator Instructions for Course Fees Workflow Procedure. Before data is entered into the online forms in the Course and Fee Management system, course and special fee forms, including a course syllabus where applicable, should be filled out and submitted to curriculum committees for review and approval. Internal Use Templates

Use the templates found on the Academic Administration website to gather the necessary information before submitting a request in UAccess. Use the completed template to fill out the UA Course Fee Forms.

A listing of all special course fees currently active in the Course Catalog can be found at the Course Fees website.
