Need COSMOS information?

The Student Information System (SIS) is a collection of systems that support the Admissions, Student Records, Financial Aid, Accounts Receivable, Advising, and Degree Audit functions of the University.

Requirements for Requesting SIS Access:

SIS access is available to University of Arizona administration, faculty and staff.

In addition to being a University employee, applicants must meet the following requirements before access to SIS can be established:

  • Complete the online FERPA Web Course. This web course reviews the federal law that governs the rights of students and institutional responsibilities with respect to student records.
  • Have the approval of their department.
  • Print, complete and submit the appropriate SIS Account Request form.

Request Process:

Completed SIS account request forms should be sent directly to SIS Accounts, Modern Languages Bldg, Room 347, PO Box 210067.

Once SIS requirements are verified, including FERPA training, the account will be created and sent to CCIT for activation. If you have not completed the FERPA Web Course, you must do so before your SIS request will be processed.

After your SIS account is activated, your userid and password will be mailed to your campus address. Along with your userid and password, you will receive information regarding the Logon & Navigation classes. Logon & Navigation is not a requirement, however, is useful when introducing new users to the SIS.

NOTE...

  • Requests for FRS access should be directed to FSO Technology at 626-3072 or go to http://uabis.arizona.edu/uabis/eforms/Launcher.asp?FormID=6
  • A new SIS account is required when you transfer to a different department.
  • SIS accounts must be deleted if:
    • You transfer to a different department,
    • You are no longer a University employee, or
    • SIS access is no longer required.
