Instructions
Before you begin filling out this form, please take note of the following:

* Only the recognized President or Treasurer may submit event request forms.
* The organization must be officially recognized by the Center for Student Involvement and Leadership.
* The officers must have submitted the Room Reservation Application Policy Form to Room and Course Scheduling prior to requesting a room.
* Two weeks notice are required for all request.

Name of President or Treasurer.
Cat card number of President or Treasurer.
Name of club/sorority/fraternity.

UA email address.


Marking Yes indicates that money will be collected in the form of admission fees, donations, or items being sold at or for the event. Marking No indicates that no money will change hands in relation to the event.
