Degree Certification Procedures for Undergraduate Students in the College of Public Health
You should apply for your degree in your department office two semesters before you expect to graduate. Students must indicate on the application whether they are ROTC or Honors College participants. Your major advisor will notify the Graduation Services Office that you have applied, and a $35 fee will be charged to your Bursar's Office account.
Your major advisor will review your course work and make any adjustments necessary to your SAPR (Student Academic Progress Report). He or she will send a Senior Degree Check Adjustment Form to Carol Carpenter, the Graduation Services Advisor for Health Professions. Carol will verify that all requirements for graduation will be met once you complete your final semester of coursework.
After your final grades come in, Carol will do a final audit of your records. If you fail to complete your degree requirements, you will be mailed notification.

HONORS PROGRAM PARTICIPANTS:
Alan Beaudrie in the College of Public Health will contact you in your final semester to retrieve your Senior Degree Check Adjustment Form from his office. You will then need to schedule an appointment with the Honors College to have them review and approve your status and sign the form, which you will then return to Alan Beaudrie.
